How Do I Reach This Student

I have a student in one of my classes that is a star on the school football team. He have letters from several colleges and he seems to have a bright future. This is the problem, he have stages where he does his work, then he goes into a stage where he does nothing. At the present time he is failing my class and a couple other classes. His mother is deceased and his father was recently released from prison. I suspect he does not have the family support he needs to be successful in school. It's no way he's going to make it in college with this kind of attitude. My question is, how do I reach this student before he ruin his college opportunities and possibly a football career. This is one opportunity I feel he cannot let get away from him. All suggestions are welcomed. Thanks.
